A new study led by the University of Warwick shows that even people who are only minimally altruistic—valuing their own life about as much as roughly 100,000 other lives—will still find it rational to dramatically cut their social contacts when they are infected. When many individuals act this way, transmission drops, outbreaks stay smaller, and in some scenarios major outbreaks can be prevented altogether.

How people decide to isolate during an outbreak
Researchers used mathematical modeling and game theory to examine how people make decisions during an epidemic. They considered factors such as infection status, concern for others, how large the outbreak is, expected time to vaccination, the disease’s transmission rate (R0), the costs of catching the disease and of social distancing, and the proportion of symptomatic cases.
Analyzing these interactions revealed two distinct epidemic outcomes, known in game theory as Nash equilibria—stable states populations settle into because individuals cannot improve their situation by changing their behavior alone.
In one equilibrium, infected individuals are altruistic enough to self-isolate aggressively, suppressing the disease long-term while allowing healthy people to continue normal activity. This is called indefinite suppression. In the other equilibrium, infected individuals choose not to isolate, leaving susceptible people to bear most of the social distancing burden and allowing the disease to spread until herd immunity develops.
What sets these equilibria apart is the level of altruism—and strikingly, the threshold needed to stabilize disease suppression can be very low, according to Warwick PhD student Mark Lynch.
Under the model, only a modest degree of concern from infected individuals can enable indefinite suppression as an alternative to herd immunity, potentially leading to far fewer infections, deaths, and social disruption.
Crucially, this result holds even when many cases are asymptomatic, some people act selfishly, or when a vaccine is anticipated.
Implications for public health policy
Some people instinctively question government guidance, wondering, “Is this really in my best interest?” The study’s authors argue that even a small amount of altruism makes self-isolation a Nash equilibrium—asking people to resist cheating and recognizing that there isn’t a better personal strategy once isolation becomes rational. It isn’t obvious to the public, and even scientists hadn’t properly understood this until now.
The findings have direct policy implications. During recent pandemics, public health messaging often urged people to stay home to protect others. This research provides a theoretical foundation for when such messages are most effective and how to improve them.
Public health officials can strengthen cooperation by communicating early and presenting isolation as both morally reasonable and rational. A key caveat: the altruism needed to sustain suppression grows as an outbreak expands—the more infections there already are, the harder it becomes to maintain indefinite suppression.
The study’s simplicity—combined with the low altruism threshold—suggests this behavior might reflect an evolved strategy in social animals to protect relatives. In many species, sick individuals reduce social interaction or leave group settings to limit transmission, which may unintentionally stabilize the group’s health.
Policy takeaway: you don’t need to invent new behavior. Messages that appeal to protecting loved ones—like “stay home to protect granny”—tap into a natural altruistic impulse.
And as the researchers put it, you should self-isolate even if you don’t like granny very much!
